2012 Ticket Order Form

Come see some of the biggest names in golf as they compete for local charities in the 2012 CVS Caremark Charity Classic, June 17-19, 2012.

To purchase tickets by mail, please print and complete our Ticket Order Form and return with payment to:

You may also place phone orders by calling 1-866-CVS-9441. All tickets will be mailed to address provided beginning in May 2012. Orders placed after June 11, 2012 will be held at Will Call. No refunds will be available.

Children 17 and under admitted free when accompanied by a ticketed adult.

Good Any Day - $25
One-day grounds admission. Valid June 17th, 18th OR 19th, 2012.

Season Pass - $60
Three Good Any Day passes.  Valid June 17th, 18th OR 19th, 2012.

2 tickets per day
2 tickets, good-any-day grounds
3 Public parking passes

INDIVIDUAL PACKAGE  - $150*
INCLUDES:
1 ticket per day or $50 per day
